The Health and Vital Statistics Office (HVSO) was established by PL15-50 (Vital Statisctics Act of 2006) and was signed into Law on March 14, 2007. The Vital Statistics Act ( specifies the scope and authority of the HVSO. The HVSO is critial in maintaining, developing, analyzing, and disseminating information used to plan and evaluate health programs and to identify specific problem areas, such as infant mortality, teen pregnancy, and Non-communicable disease. 

Vital Records: 

Vital records and/or vital events are defined as birth, death, fetal deaths (stillbirth), and other related reports.  


The mission of the Commonwealth of the Northern Mariana Islands (CNMI) Office of Health and Vital Statistics is to document/provide accurate records and data concerning vital events to the Government of the Northern Mariana Islands and other Stakeholders in an expeditious and friendly manner. 


CNMI Vital Records are closed public records. We comply with Health Insurance Portability and Accountability Act of 1996 (HIPAA Privact Act). Protected Health Information (PHI) are priviledged information and are strictly protected under the HIPAA Privacy Act and CNMI Public Law 15-50. 

Birth records

Vital records are confidential. Birth records are available from 1930 to the present. Health & Vital Statistics Office requires that all requests for vital records include valid picture ID of the requestor. Acces to birth records is limited to: 

  • The person named on the certificate; 
  • The parents shown on the birth record;
  • Immediate family members;
  • Authorized legal guardian or agent (authorization letter)
  • Person who has been granted a court order

Death records

Vital Statistics duty of confidentiality continues after a patient has died. Death certificate containing the cause of death information shall not be issued except as follows: 

  • Upon specific request of the spouse, children, parents, or other next of kin of the decedent or their respective authorized representatives ingredients;
  • When a document needed for the cause of death to establish a legal right or claim has been demonstrated;
  • When the request for the copy is made by or on behalf of an organization that provides benefits to the decendent's survivors or beneficiaries.    

PRIMARY SERVICES:Provide services to walk-in customers

  • Prepare and mail certified copies of birth and death records
  • Record and enter all occurences of vital events into electronic systems 
  • Prepare delayed certificates of birth and death, amend birth and death records, complete new birth certificates upon receipt of adoption orders, and process legitimation and paternity acknowledgments.
  • Provide statistical reports on CNMI vital events to management & other stakeholders


Birth Certificate:

  • On-island: $25.00 each 
  • Off-island: $25.00 each + $5.00 (mail services flat fee) 
  • Amendment: $15.00 

Death Certificate: 

  • On-island: $20.00 each
  • Off-island: $20.00 each + $5.00 (mail service flat fee) 
  • Amendment: $15.00
